The Oklahoma Department of Environmental Quality was informed of an overexposure of a radiographer employed by Tulsa Gamma Ray. The total dose for the year to date involved is 67 mSv (6.7 rem). It is unknown at this time whether the exposure was acute or chronic. More details will be provided as they become available.
UPDATE 04 JUNE 2013: it has been determined that the final dose is 67 mSv (6.7 rem) and the dose was an acute overexposure. The overexposure occurred because the radiographer cranked out the source, became distracted, and forgot to crank the source back inside the camera. Also, he stated that he had not seen the warning lights because the brim of his safety helmet blocked them, and did not hear the audio alarm. It was later determined that the warning lights and audio alarm had been functioning properly. Corrective actions included an engineering change to the system. NRC EN48836.
