HUMAN ERROR INITATED REACTOR TRIP

The reactor was scrammed as a result of the "ECCS hydroaccumulators water level drop" spurious signal. The signal has been generated because an operator erroneously closed the valve on instrumental line of wrong water level sensor. Closing of instrumental line valve was necessary to isolate and fix a leak in instrumentaiton system's piping of the other sensor. The leak has been detected by routine walk-down. The operator's mistake has been caused by wrong inscription on the sensor.
There was no release of radioactive material outside of the reactor facility, neither radation exposure to the personnel.
Justification of rating:
Preliminarily the event was rated at level "0". (See ERF of 31 January 1995). The event was rated under the degradation of defence in depth criterion. "Expected" initiator (reactor automatic shutdown) occurred, but the safety function availability was regarded as "full" during the event. The box A1 on Table II gave the basic rating of level "0".
After examining of NPP's Event Investigation Report and discussions with NPP staff, the event has been uprated to level 1 due to procedural inadequacies and safety culture factors according to section II.5.2 and III.5.3. The factors' using is justified by the event root causes: shortcomings to procedure on instrumentation equipment walk-downs and deficiency in the quality assurance process.
