NON-AVAILABILITY OF CHANNEL TEMPERATURE MONITORING (CTM) INSTALLATION-2

Unit was operating at 180 MWe. All the readings on CTM panel II were found to be zero. The 36 V DC power supply failure on CTM panel-II led to non-availability of setback as the setback is initiated on 2 out of 2 coincidences from the setback signals generated in panel-I & panel II. Immediately reactor power was lowered by 10 MWe. A close vigilance was kept on CTM panel-I to initate the manual setback in the event of very high temperature on panel-I or trip the unit if required. However, CTM panel-I readings were normal and remained so during the 53 minute duration for which 36V DC power supply was lost. Existing design of the circuit with respect to 36V DC power supply is not fail safe since failure of power supply does not result in CTM installation going to setback mode.
Justifiaction: 36 V DC power supply failure to CTM panel-II has not been taken as the initiating event as it did not challenge any of the safety functions. However, flow blockage in an individual channel has been taken as an expected initiator which did not occur. So Table-I of the Users Manual has been used for rating of the event. Since safety function availability was full the event is rated at level 0 as per User's manual TABLE-I BOX AI.
