REACTOR AUTOMATIC SHUTDOWN DUE TO MISOPERATION BY AN OPERATOR

When the plant was at the preparation stage of power generation after plant having been started-up, an operator changed the reactor mode switch from "start-up" to "operation" at the lower pressure of main steam than set-point value, subsequently the reactor was automatically shut down.
This plant had been manually shut down on November 20, 1990 because of an alarm "Primary Loop Recirculation Pump Motor Lubrication Oil Level Low".
There was no release of radioactive materials outside the reactor facility. Also there was no unplanned occupational exposure of employees working under radiation.
BASIS FOR RATING: The misoperation by an operator was a simple careless mistake. Thus alevel 0 of INES can be selected for this event by the defence-in-depthcriterion.
